Job Summary

The Social Worker MSW Palliative Care is a critical member of our Palliative Care team, providing expert psychosocial support to patients and their families. This role requires a strong understanding of palliative care principles and a commitment to delivering compassionate, patient-centered care.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify patient and family needs
  • Develop and implement individualized care plans to address patient and family needs
  • Provide expert psychosocial support to patients and their families, including crisis intervention, counseling, and education
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Stay up-to-date with community-based resources and services to support patients and families
Requirements
  • Master of Social Work (MSW) degree from an accredited institution
  • Current licensure as a Clinical Social Worker (LCSW) preferred
  • Minimum of 2 years clinical experience, or Palliative Care intern training
  • Specialized training/certification in Palliative Care, or related area preferred
