We are seeking a skilled and compassionate Physical Therapist to join our team in a home health setting. As a Physical Therapist with AccentCare, you will have the opportunity to work with patients in the comfort of their own homes, providing personalized care and support to help them achieve their goals.

Key Responsibilities
  • Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans to address their needs.
  • Provide one-on-one physical therapy sessions to patients in their homes, using evidence-based techniques and modalities to promote healing and recovery.
  • Collaborate with patients' healthcare teams to ensure seamless care coordination and communication.
  • Document patient progress and outcomes, using data to inform treatment decisions and improve patient outcomes.

Requirements
  • Master's degree in physical therapy from an accredited program.
  • Current licensure to practice as a physical therapist in the state of operation.
  • Minimum nine months of experience as a physical therapist, with a preference for community or home health experience.
  • Valid CPR certification.
  • Licensed driver with a reliable vehicle for travel to patient locations.
