Key Responsibilities- Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans to address their needs.
- Provide one-on-one physical therapy sessions to patients in their homes, using evidence-based techniques and modalities to promote healing and recovery.
- Collaborate with patients' healthcare teams to ensure seamless care coordination and communication.
- Document patient progress and outcomes, using data to inform treatment decisions and improve patient outcomes.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Master's degree in physical therapy from an accredited program.
- Current licensure to practice as a physical therapist in the state of operation.
- Minimum nine months of experience as a physical therapist, with a preference for community or home health experience.
- Valid CPR certification.
- Licensed driver with a reliable vehicle for travel to patient locations.
