### Job Responsibilities

* Assess, plan, implement, and evaluate the nursing care for a caseload of clients from admission to discharge
* Coordinate and supervise the nursing care delivered by home health aides
* Assume case management responsibilities as assigned
* Coordinate medication administration based on individual need
* Assess for significant changes in a client's condition and update care plans accordingly
* Supervise home health aides as assigned
* Initiate and complete necessary documentation and reporting
* Communicate order and status changes to physicians
* Maintain a safe environment for clients and employees

### Qualifications

* Valid driver's license and safe driving record
* Registered Nurse from an accredited school of nursing with a current Ohio license
* One year of home health care experience preferred
