Job Title: Travel Contract

We are seeking a skilled and compassionate Registered Nurse to join our team as a Travel Contract RN. As a Travel Contract RN, you will have the opportunity to work with a diverse range of patients and healthcare teams, providing comprehensive care and support to those in need.

Job Summary:

This is a per diem opportunity for a Registered Nurse to work as a Travel Contract RN. The ideal candidate will have experience in case management and a strong background in geriatric care. As a Travel Contract RN, you will be responsible for providing high-quality patient care, collaborating with healthcare teams, and ensuring seamless transitions of care.

Responsibilities:
  • Provide comprehensive case management and care coordination for patients with complex needs
  • Collaborate with healthcare teams to develop and implement individualized care plans
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions of care
  • Participate in ongoing education and training to stay current with best practices and industry developments
Requirements:
  • Active, unrestricted licensure as a Registered Nurse in a state
  • Graduate from an accredited school of nursing
  • Current state's driver's license with the ability to travel to hospitals, skilled nursing facilities, patient's homes, and other sites where patients receive care as needed
  • Full COVID-19 vaccination is an essential job function of this role
Preferred Qualifications:
  • Certification in Geriatric Nursing or Geriatric Case Management
  • 2+ years of nursing experience, preferably with older patients
  • Proficient in computer use, the Internet, and health information technology
  • Demonstrated excellent communication, interpersonal, and organizational skills
