Key Responsibilities- Develop and implement a palliative care program that meets the needs of patients and families
- Provide education and support to healthcare team members on palliative care principles and practices
- Collaborate with interdisciplinary teams to ensure seamless care delivery
- Conduct regular assessments and evaluations to ensure patient needs are met
- Develop and maintain relationships with community resources and organizations to support patient care
- Current Texas Registered Nurse licensure or Compact State Registered Nurse licensure
- BCLS certification
- Associate's or Bachelor's Degree in Nursing
- Experience in hospice and palliative care nursing, with a focus on leadership, education, team building, program development, and interdisciplinary collaboration
- RN from an accredited school of nursing
- BSN preferred
- Certified Palliative and Hospice Care Nurse (CHPN) preferred
- Critical care experience in a hospital setting
