Position Overview
The Clinical Liaison Nurse, reporting to the Director of Patient Care Services, employs the nursing process to create, execute, and assess outcomes for specific patient demographics.
This role leverages a diverse skill set to educate, advise, and support patient management within a targeted patient group, aiming for optimal quality and outcomes while maintaining cost efficiency.
The Clinical Liaison Nurse collaborates with a multidisciplinary patient care team, which includes physicians, nursing staff, rehabilitation therapists, respiratory therapists, pharmacists, dietitians, and other relevant healthcare professionals. This collaboration focuses on identifying opportunities for enhancing patient care and outcomes, as well as reducing costs based on current research and evidence-based practices.

Qualifications
Required Education
Graduate of an accredited nursing program.
Preferred Education
Bachelor of Science in Nursing.
Licensure and Certifications Required
Active nursing license in the state of practice.
Preferred Licensure and Certifications
Membership in a relevant professional organization.
Specialty certification.
Required Experience
A minimum of ten years in the care of the designated patient population.

Key Responsibilities
Clinical Expertise:
Coordinates and delivers direct patient care as necessary, working alongside physicians to promote health and prevent illness.
Responds to requests from healthcare team members regarding complex patient needs.Adjusts care plans to align with individual patient requirements, particularly in specialty areas.
Engages patients and their families in goal-setting and communicates effectively about care plans and discharge objectives.
Participates in discharge planning.Regularly collaborates with physicians to discuss patient progress and needs.
Conducts rounds with providers to review patient advancements.
Works closely with Directors to evaluate supplies and equipment that support service lines.
Partners with utilization managers to identify clients within service lines, assess needs, plan interventions, implement activities, and evaluate outcomes that surpass client expectations.
Leads huddles on nursing units to anticipate and address discharge barriers.Escalates discharge barriers as necessary.
Ensures the care team is accountable for meeting the anticipated discharge date for patients.
Performs additional related duties as assigned.
Research Participation:
Facilitates the development of clinical pathways based on scientific research findings.
Monitors performance data and significant outcomes to evaluate current clinical practices against research literature.
Assumes a leadership role in quality improvement initiatives for specific patient populations using a multidisciplinary approach.
Contributes to clinical research relevant to designated patient groups.
Educational Resource Development:
Identifies educational and marketing needs, coordinating with the patient care team to create programs and materials for patients, families, and the community.
Collaborates with staff to enhance necessary skills and develop appropriate documentation tools.Enhances knowledge through participation in continuing education programs, focusing on age-related specialty education and leadership development.
Works with Directors to identify staff learning needs and provide instructional support as needed.Collaborates with relevant customer groups in strategic educational planning for the organization, community, and business partners, offering instructional support and clinical expertise as appropriate.
Administrative Responsibilities:
Promotes the mission, purpose, objectives, and strategic planning of Atrium Health.
Collaborates with the healthcare team through performance improvement processes to enhance efficiency, cost-effectiveness, and clinical outcomes.
Assists in the evaluation and revision of policies, procedures, and standards of care in cooperation with the healthcare team.
Age-Specific Care:
Communicates effectively with patients and families across all age groups, including neonatal, pediatric, adolescent, adult, and geriatric populations.
Ensures an age-appropriate environment, education, and procedures in the area of specialty.

Frequent exposure to communicable diseases, bodily fluids, toxic substances, and other conditions typical in a clinical setting.
Typical Physical Demands:
Full range of motion, including the ability to handle and lift patients, manual dexterity, and eye-hand coordination. Standing and walking for extended periods. Occasionally lifts and carries weights up to 50 pounds.
Ability to perceive letters, numbers, and symbols with normal vision and hearing to respond to alerts and equipment alarms.
