Key Responsibilities:
  • Receive patient assignments from Operations Leader or Clinical Resource Team Leader.
  • Communicate effectively regarding patient care, business operations, and positive work environment.
  • Assume responsibility for patient and family care, including assessment, planning, implementation, and evaluation.
  • Initiate and maintain communication with attending physicians, hospice staff, and other agencies for optimal patient care and resource utilization.
  • Collaborate regularly with Operations Leader or designated personnel to review care strategies.
  • Maintain consistent communication with attending physicians about patient and family care.
  • Collect data on physical, psychological, social, and spiritual factors influencing patient and family health, and integrate this information into the care plan.

Requirements:
  • Minimum of two years of nursing experience preferred; one year in home care is highly desirable.
  • Current BLS Certification.
  • Valid Registered Nurse licensure in the state of South Carolina.
  • Valid driver's license and proof of vehicle insurance.
