Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team in Glen Mills, Pennsylvania.

Job Summary:

We are looking for a dedicated and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote independence, safety, and overall well-being.

Responsibilities:
  • Conduct thorough assessments and evaluations to identify client needs and develop effective care plans.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to improve client mobility and function.
  • Collaborate with nursing and other healthcare professionals to ensure seamless care coordination and communication.
  • Educate clients and their families on rehabilitative care and activities necessary to promote independence and safety.
  • Accurately document client progress and care plans, utilizing a state-of-the-art touch pad tablet.
Requirements:
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
