Key Responsibilities:
- Engage in discussions regarding the mission, values, and services provided by Pathways Home Health and Hospice.
- Conduct thorough nursing assessments, documenting patient conditions, care interventions, and outcomes in the electronic health record.
- Utilize electronic devices for real-time documentation during patient visits, ensuring compliance with regulations.
- Maintain regular communication with the RN case manager regarding patient care and any changes in condition.
- Educate and support patients, families, and caregivers on care processes and interventions.
- Participate in interdisciplinary team meetings and training sessions as required.
- Honor the dignity of patients and respect their unique spiritual and cultural beliefs.
- Deliver holistic, family-centered care across various treatment environments to enhance quality of life.
- Implement non-pharmacologic interventions to promote comfort and manage distress.
- Contribute to quality improvement initiatives and participate in relevant educational opportunities.
- Adhere to established productivity standards and agency policies.
- Ensure accurate documentation of visits and maintain communication with all stakeholders involved in patient care.
- Stay informed about safety and infection control protocols within the care setting.
- Continuously update knowledge and skills necessary for providing exceptional hospice nursing care.
- Monitor and manage medication lists, educating patients and families on proper usage and potential side effects.
- Promote a culture of service by treating all individuals with respect and professionalism.
- Assist in the development of hospice care protocols and policies as needed.
- Perform additional duties as assigned that align with the mission and goals of Pathways.
- Active California LVN License.
- Preferred certifications in IV Therapy and Blood Withdrawal.
- At least one year of LVN experience within the last three years.
- Strong motivation, self-direction, and the ability to collaborate effectively within a multidisciplinary team.
- Excellent assessment, communication, and problem-solving abilities.
- Valid California driver's license and automobile insurance as per agency policy.
- Current CPR certification from the American Heart Association.
