MDS Nurse

We are seeking a highly skilled and dedicated MDS Nurse to join our team at SLP Operations, LLC. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality of care for our residents.

• Manage the Resident Assessment Instrument (RAI) process from admission to discharge, ensuring compliance with federal and state guidelines.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Coordinate resident assessments and services with other clinical departments.
• Provide timely information for managed care clinical authorization and justifications.
• Lead daily PPS/OBRA case management meetings and weekly level of care meetings.

• Current Registered Nurse (RN) or Licensed Vocational Nurse (LVN) licensure in good standing.
• Minimum Data Set and Care Plan experience preferred.
