Hosparus Health is committed to delivering compassionate care to our patients and their families. As a Social Worker, you will play a vital role in our interdisciplinary team, providing emotional support, assistance with financial and social service needs, and guidance on end-of-life decisions.
Key Responsibilities:- Assess patient needs and develop personalized care plans
- Provide emotional support and counseling to patients and families
- Assist with financial and social service needs, including benefits navigation and resource referral
- Support patients and families in making informed end-of-life decisions
- Collaborate with healthcare teams to ensure comprehensive care
- Licensed Social Worker designation in Kentucky with a baccalaureate degree in social work or social welfare program accredited by the Council on Social Work Education
- Minimum one-year social work experience in healthcare
- Technology proficiency, including Microsoft Suite, Electronic Medical Record, and videoconferencing
- Excellent communication and problem-solving skills
- Ability to work independently and adapt to diverse client needs
