VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our homecare team in Lyndhurst, New Jersey.
Job Summary:We are looking for a dedicated and experienced RN to provide high-quality patient care in the comfort of their own homes. As a Home Health RN, you will be responsible for assessing patients, developing care plans, and collaborating with healthcare teams to deliver comprehensive care.
Responsibilities:- Assess and evaluate patients with multiple health conditions and develop appropriate care plans.
- Document changes in patient conditions and update care plans and outcomes.
- Identify teaching needs and plan strategies for patients and families.
- Monitor the quality and appropriateness of services provided.
- Collaborate with healthcare providers and multidisciplinary teams to deliver comprehensive patient care.
- Adhere to agency infection control and safety policies.
- Maintain the organization's standards for attendance and professional appearance.
- RN with Bachelor's degree in Nursing preferred
- 1+ year of supervised clinical experience as a registered nurse
- Excellent verbal and written communication skills
- Strong computer skills
- Excellent organizational and interpersonal skills
- Car and valid driver's license
