Job Summary

We are seeking a highly skilled and experienced RN Patient Care Manager to join our team in South Austin. As a RN Patient Care Manager, you will be responsible for managing a team of healthcare professionals and overseeing the delivery of patient care services in the home health setting.

Key Responsibilities
  • Manage a team of healthcare professionals, including RNs, LPNs, and CNAs, to ensure high-quality patient care and efficient delivery of services.
  • Develop and implement patient care plans in collaboration with the interdisciplinary team.
  • Conduct regular patient assessments and evaluations to ensure patient needs are met and care plans are updated as needed.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination and communication.
  • Monitor and manage patient outcomes, including hospital readmissions and patient satisfaction.
  • Ensure compliance with regulatory requirements, including Medicare Conditions of Participation and federal, state, and local regulations.
Requirements
  • Current licensure as a Registered Nurse in the practicing state.
  • Minimum of 2 years of experience in home health care, with at least 1 year of management or supervisory experience.
  • Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements.
