Job Summary

We are seeking an experienced Physical Therapist to join our outpatient rehabilitation team at Lakeview Care Center. As a Physical Therapist, you will play a vital role in providing high-quality therapy services to patients, helping them recover from injuries, illnesses, or surgeries.


Key Responsibilities:
  • Conduct thorough evaluations to assess patient needs and develop personalized treatment plans.
  • Provide evidence-based physical therapy interventions to restore function, relieve pain, and improve mobility.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and effective communication.
  • Maintain accurate and detailed records of patient progress and treatment outcomes.
  • Participate in ongoing education and professional development to stay current with best practices and advancements in the field.

Requirements:
  • Bachelor's degree or equivalent from an accredited physical therapy program.
  • 6 months of relevant experience in outpatient rehabilitation settings.
  • Licensure as a Physical Therapist in the State of Michigan (LPT-MI).
  • Basic Life Support (BLS) certification from American Heart Association or American Red Cross.
