Case Manager

We are seeking a highly skilled and dedicated Case Manager to join our Child Welfare team. As a Case Manager, you will be responsible for performing case management in the Child Welfare area of Investigations, conducting timely initial investigations of reports of child abuse and neglect, and offering services to children and families who may have experienced or are at risk of experiencing child maltreatment.

- Perform case management, including contacts, visitation, and case planning, to ensure safety, permanency, and well-being of children and families.
- Respond to assigned intakes within identified time frames to conduct initial safety assessments of the family and home using the comprehensive Family Advocacy and Support Tool (FAST).
- Develop safety plans to reduce safety concerns identified in FAST assessments.
- Authentically engage families, other professionals, and external partners with empathy and respect, and collaborate closely with law enforcement and legal when safety concerns are present.
- Make concerted efforts to locate and engage absent parents, relatives, and fictive kin to support and join the child and family team (CFT).
- Participate in initial CFT meetings and ensure youth and families feel heard and have a voice in the teaming process.
- Recommends intervention services or activities to strengthen the family and safely maintain the child(ren) in the home based on the initial assessment of safety, and makes timely service referrals.
- Maintain physical and electronic case records by composing clear, objective, and thorough documentation covering pertinent details of case activities per DSS policy and the GPS Practice Model.
- Maintain child welfare certification and complete 20 hours of training yearly to include certifications that are required yearly.
- Maintain an active routine of self-care and pursue support when necessary to minimize the threat of secondary trauma to physical and mental health.
- Perform Red Cross shelter duty or other emergency shelter duties/services as directed.
- A Bachelor's Degree in Social Work, Behavioral Science, or Social Science, or a Bachelor's Degree in any other field and one (1) year experience in a related field.
- Valid driver's license, ability to drive, and willingness to transport children and adults.
- Ability to lift children and interact with children and adults in a positive manner.
- Ability to work overtime hours, including weekends and during natural disasters or deployments.
