Job Summary:
We are seeking an experienced Nurse Practitioner to provide primary care services to older adults on Medicare. This role involves conducting annual wellness visits, health risk assessments, and population health leadership. The ideal candidate will have a passion for preventative care, excellent communication skills, and a commitment to delivering exceptional patient experiences.
Responsibilities:
- Conduct annual wellness visits and health risk assessments to identify patients' needs and develop personalized care plans
- Provide population health leadership, ensuring eligible females receive evidence-based breast cancer screenings every two years
- Collaborate with the Care Team to promote healthy behaviors and disease prevention
- Administer home visits as needed to address patients' specific needs
- Stay up-to-date with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs)
Benefits:
- A competitive salary of $120,000 - $150,000 per year, based on experience

Requirements:
- Bachelor's degree in Nursing or related field
- Master's degree in Nursing (MSN) or Doctoral degree in Nursing (DNP) preferred
- Board certification as a Nurse Practitioner (NP)
- At least 2 years of experience in primary care settings
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
