Job Description

Job Summary:

  • Provide high-quality physical therapy services to clients in their homes.
  • Develop and implement individualized care plans to promote client independence and well-being.
  • Collaborate with multidisciplinary healthcare teams to ensure comprehensive care.
  • Conduct thorough assessments and evaluations to identify client needs and goals.
  • Implement manual therapeutic exercises, gait training, balance, and other interventions to promote client recovery.

Responsibilities:

  • Make home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements to assess client mobility, strength, and balance.
  • Develop and implement individualized care plans to promote client independence and well-being.
  • Continually assess and revise care plans to ensure optimal client outcomes.
  • Accurately document client information, interventions, and evaluations.

Requirements:

  • A current Physical Therapist license in the state.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
