This team comprises both clinical and non-clinical professionals who work together to ensure high-quality, patient-centered care. The Rehabilitation Specialist plays a vital role within the IDT, engaging directly in care planning and working alongside other clinical team members.
The Rehabilitation Specialist is responsible for organizing and delivering restorative and rehabilitative therapy services to participants, both at our facility and in their homes. Collaborating closely with the Primary Care Physician, rehabilitation team, and other IDT members, the Rehabilitation Specialist aims to enhance participant independence and safety while focusing on the development or restoration of functional abilities.
Key Responsibilities:
- Perform comprehensive therapy assessments in our facility and at participants' homes to identify their therapy needs, preferences, and objectives.
- Actively engage in IDT meetings to formulate participant care strategies.
- Implement and document therapy interventions in line with physician directives and/or as outlined in the participants' care plans.
- Administer both active and passive exercises, muscle re-education, gait training, and functional training for activities of daily living (ADLs), utilizing various treatment modalities such as massage, heat, sound, water, light, or electrical stimulation.
- Oversee the Therapy Assistants, providing necessary training, instruction, and guidance to ensure the effective implementation of Functional Maintenance Programs (FMP).
- Educate staff, participants, and caregivers/families on the use and maintenance of therapeutic equipment.
Qualifications:
- Bachelor's Degree in Physical Therapy with a minimum of five (5) years of relevant experience.
- Master's Degree or Doctorate in Physical Therapy is preferred.
- Possession of a valid California Physical Therapy License or proof of eligibility for licensure.
- At least two (2) years of experience in a clinical setting, particularly with a frail or elderly demographic, is preferred.
- Internationally earned physical therapy degrees must be evaluated for equivalency to US standards.
- Current CPR certification is preferred.

Salary range for this position is $99,309 - $119,171 annually, plus bonuses and equity. WelbeHealth offers a competitive total rewards package that includes a 401k match, healthcare coverage, and a variety of other benefits. Actual compensation will be adjusted based on experience and qualifications.
