- Ensure meticulous and current documentation of nursing care delivered, facilitating the integration of information for the healthcare team.
- Conduct initial assessments upon patient admission and formulate a suitable care plan that adheres to unit standards and reflects patient and family requirements.
- Implement the care plan, nursing interventions, and patient care protocols.
- Adjust the care plan based on ongoing evaluations.
- Prioritize patient and employee safety by identifying and addressing safety hazards and incidents.
- Engage in direct communication with colleagues to resolve issues privately when necessary.
- Provide constructive feedback to support the professional growth of peers and collaborate with colleagues to enhance knowledge.
- Recognize personal learning needs and proactively seek appropriate solutions.
- Meet all departmental competency requirements.
Department-Specific Duties:
- Receive patient Bedside Shift Report from the preceding RN at the start of each shift, ensuring patient input is included, verifying a safe patient room environment, and updating the patient communication board in each room.
- Delegate patient care tasks to assigned Healthcare Technicians (HCTs) and confirm the completion of these tasks throughout the shift.
- Document essential RN patient care activities, including assessments, vital signs interpretation, ensuring physician orders are fulfilled, managing patient admissions/discharges, and conducting hourly rounds on each assigned patient.
- Adhere to documentation standards set by the organization for each assigned patient.
- Administer patient medications following the 5 Rights: Right Patient, Right Medication, Right Route, Right Dose, and Right Time.
- Maintain patient safety as the utmost priority, ensuring compliance with safety measures such as fall precautions, pressure ulcer prevention, isolation protocols, and safe patient ambulation.
- Exhibit a consistently positive attitude on the unit to foster a healthy work environment and promote patient healing.
- Maintain a strong focus on promptly responding to patient call lights to ensure effective triage and address patient requests and medical needs.
- Utilize critical thinking to assess overall patient conditions and apply nursing judgment to identify and respond to changes in patient status.
- Communicate effectively within the healthcare team to enhance interdisciplinary collaboration, coordinate patient care, and ensure patient safety.
- Efficiently manage time to facilitate the swift admission and discharge processes for Observation status patients and expedite patient testing to support timely discharge decisions.
