Job Description

At Kindful Health LLC, we are seeking a skilled Nurse Practitioner Hospice to join our team. As a key member of our interdisciplinary group, you will play a vital role in ensuring the highest quality care for our patients and their families.

  1. Collaboration and Assessment: Collaborate with the Interdisciplinary Group (IDG) to ensure accurate evaluation and palliative treatment of terminal disease, utilizing your expertise to develop and implement effective care plans.
  2. Plan of Care Development: Work closely with the IDG to create comprehensive patient/family plans of care, guaranteeing that our patients receive the best possible care.
  3. Visits and Documentation: Perform and document visits to patients and their families, as outlined in the plan of care and based on their individual needs.
  4. Interdisciplinary Collaboration: Interact with attending and consulting physicians as necessary, working in collaboration with our hospice physician to ensure seamless care coordination.
  5. Face-to-Face Encounters: Complete face-to-face encounters with patients, as assigned, prior to the third benefit period and each subsequent period, to ensure timely and effective care.
  6. Clinical Assessment and Reporting: Provide clinical assessment results to the hospice physician after making face-to-face encounters with patients, as assigned, to inform care decisions.
