Job Overview:As an In-Home Care Specialist at Easy Health, you will play a crucial role in delivering preventive care and educational services directly to our members in their communities.
You will perform comprehensive assessments, including Annual Wellness & Quality Gap Care visits for our members in their homes.
This is an excellent opportunity to make a meaningful difference in the lives of our elderly and chronically ill members.
Responsibilities:- Conduct clinical screenings and various tests in an in-home setting.
- Accurately code, verify, and document clinical findings.
- Complete a detailed evaluation of the medical and social assessment of our beneficiary members using our proprietary tool or client EMR.
- Identify and refer members/patients for routine, urgent, and emergent case management.
- Makes recommendations for members' follow-up with their primary care physician.
- 2 years of hands-on experience as a Nurse Practitioner.
- Current, unrestricted license in the state(s) in which the services are rendered.
- Board-certified, preferably geriatric or adult medicine specialty.
- Familiarity with the HCC model and quality HEDIS and other STAR measures.
- Effectively communicate with elderly and chronically ill members.
- Proficient in the use of computer, tablet, and mobile technology.
The estimated annual salary for this position is $80,000 - $120,000 based on performance and location. Payment for same-day no-show visits is also included.
As an independent contractor, you will have the opportunity to earn a reimbursement for travel mileage.
