As a Home Health Nurse, you will provide high-quality patient care and support to individuals in their homes, working closely with our multidisciplinary team to develop and implement personalized care plans.
Key Responsibilities- Provide comprehensive patient assessments and develop individualized care plans to meet the unique needs of each patient.
- Deliver high-quality patient care and support, including medication management, wound care, and education on disease management and self-care.
- Collaborate with our multidisciplinary team, including physicians, therapists, and social workers, to ensure seamless care coordination and communication.
- Conduct regular patient visits and assessments to monitor progress and make adjustments to care plans as needed.
- Document patient information and progress in a timely and accurate manner, ensuring compliance with regulatory requirements.
- Graduate of an accredited Registered Nurse (RN) program.
- Active, unrestricted Registered Nurse (RN) license in the State of South Carolina or compact multi-state license.
- Minimum of one year of nursing experience, ideally in home healthcare, Med/Surg, or Critical Care.
