Job Summary:

HexaQuEST Health, Inc. is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will be responsible for treating patients with movement dysfunction or pain to improve their ability to perform daily tasks, decrease risk of adverse events, and improve patient outcomes.

Key Responsibilities:

  • Treat patients with movement dysfunction or pain to improve their ability to perform daily tasks, decrease risk of adverse events, and improve patient outcomes.
  • Document all patient care activities, including evaluations, progress notes, discharges, daily notes, insurance approvals/denials, and phone conversations.
  • Communicate effectively with other healthcare providers, insurance companies, patients, and rehabilitation staff.
  • Perform cleaning and maintenance tasks as needed.

Requirements:

  • Active state license as a Physical Therapist.
  • Less than 1 year of experience.
  • 36 minimum guaranteed hours.

Working Conditions:

This is a full-time position with possible weekend shifts. The ideal candidate will be able to work independently and as part of a team to provide high-quality patient care.
