We're seeking a highly skilled Pediatric Care Navigator to lead our team in Tyler, TX.

About the Role:

Your Key Responsibilities:

  • Collaborate with multidisciplinary teams to develop and implement comprehensive care plans that address the unique needs of each patient.
  • Utilize advanced nursing skills and knowledge to navigate complex healthcare systems, identifying opportunities for process improvements and promoting best practices.
  • Advocate for patients and families, addressing their concerns and empowering them to take an active role in their care.
  • Develop and maintain strong relationships with healthcare providers, social workers, and other stakeholders to ensure coordinated care.
  • Stay up-to-date with industry trends and best practices, applying this knowledge to enhance patient outcomes and drive quality improvement initiatives.

The Ideal Candidate:

We're looking for a seasoned nurse with at least 4 years of experience in pediatric nursing, case management, or a related field. You should possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

What We Offer:

We provide a comprehensive compensation package, including a competitive salary of $93,000 - $114,000 per year, depending on experience. Additionally, we offer a range of benefits, including medical, dental, and vision coverage, as well as opportunities for professional growth and development.
