Key Responsibilities- Assess patient health care preferences and expectations
- Collaboratively coordinate the continuum of care across settings
- Develop patient care plans based on patient condition, age, and psychological, educational, and socioeconomic conditions
- Implement patient care plans according to patient priorities and document interventions and patient/family response
- Collaborate and integrate contributions of patients and families into multidisciplinary patient care plans
- Develop, coordinate, and implement transfer/discharge planning throughout the episode of care
- Current license to practice nursing in Iowa
- A minimum of one year RN telemetry experience or one year of RN post-surgical step-down experience
- Excellent written and verbal communication skills
- General computer skills
- Baccalaureate Degree in Nursing or Masters Degree in Nursing
- Active enrollment in a BSN program
- A competitive salary range of $54,530 to commensurate
