Responsibilities- Provide high-quality, patient-centered care to patients in their homes
- Conduct thorough assessments and develop individualized care plans
- Collaborate with the healthcare team to ensure effective care coordination
- Document patient progress and outcomes accurately and timely
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments
- Active Professional licensure within the state
- 1 year of recent experience within the Home Health specialty
- OASIS & HCHB experience required
- BLS/ACLS Certification from the American Heart Association
