Job Description:
We are seeking a skilled Nurse Practitioner to join our integrated care delivery program in Dover, New Hampshire. The ideal candidate will have a strong background in managing acute and chronic medical and behavioral conditions, as well as excellent communication and collaboration skills. You will be responsible for conducting comprehensive assessments of patients, communicating with the interdisciplinary care team, and providing patients and caregivers with counseling and education on various health topics.

Responsibilities:
  • Conduct comprehensive assessments of patients, including advanced illness and advanced care planning conversations.
  • Manage acute and chronic medical and behavioral conditions effectively.
  • Communicate and collaborate with the interdisciplinary care team.
  • Provide patients and caregivers with counseling and education on various health topics.

Requirements:
  • An active and unrestricted license in New Hampshire.
  • A graduate degree from an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following areas: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner.
  • Current DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs.

Salary: $70-$85 per hour.
