**Job Summary:**
We are seeking a skilled Care Coordination Specialist to join our team at HC&N Healthcare Solutions. As a key member of our interdisciplinary team, you will play a vital role in ensuring the needs and expectations of our residents are met during their stay and beyond.
**Key Responsibilities:**
* Collaborate with the care team to develop and implement comprehensive care plans that meet the unique needs of each resident.
* Work closely with residents and their families to coordinate services and support, ensuring a seamless transition from our facility to their community.
* Partner with the finance team to ensure accurate documentation and billing processes are in place.
* Develop and maintain relationships with community resources and service providers to ensure residents receive the necessary support upon discharge.
**Requirements:**
* Minimum 2 years of experience in social work or a related field.
* Bachelor's degree in Social Work, Healthcare, Psychology, or a related field. Master's degree not required.
* Proven experience working in long-term care settings, with sub-acute experience a plus.
* Ability to work flexible schedules and adapt to changing priorities.
* Proficiency in electronic medical records (EMRs).
