Key Responsibilities
  • Implement and manage individualized care plans for patients with complex medical needs
  • Conduct thorough patient assessments and coordinate care with healthcare teams
  • Administer medications, treatments, and therapies as prescribed by physicians
  • Document patient progress and maintain accurate records
  • Provide education and support to patients and their families on disease management and self-care
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
Requirements
  • Active, unencumbered license as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN) in the state of practice
  • Current CPR certification from a recognized organization
  • TB skin test (current within the last 12 months)
  • At least six months of prior hands-on nursing experience, preferably in home health care
  • Reliable transportation and a valid driver's license
