Primary Care Nurse Practitioner

In collaboration with a physician at VNA Health Center, the Family Nurse Practitioner delivers thorough medical assessments, education, and management for patients across various stages of life seeking preventive, acute, and chronic care services at VNA Health Center locations.
- Evaluates and interprets medical and psychosocial histories; conducts physical examinations, including gynecological assessments as appropriate; identifies health issues; develops treatment plans; provides education and referrals as necessary; and assesses the effectiveness of treatment and education.
- Collaborates with or refers patients with abnormal findings or other medical, psychological, or social concerns to the supervising physician or suitable referral sources when necessary.
- Orders, performs, and interprets laboratory tests as part of health assessments and management. Reports communicable diseases promptly to the relevant health department in accordance with established protocols.
- Manages conception and contraception in accordance with applicable protocols and guidelines for family planning services, including confidential services for adolescents, pregnancy testing, options counseling, emergency contraception, and consideration of all FDA-approved contraceptive methods.
- Engages in health education and counseling during physical examinations, empowering clients to make informed decisions regarding their health and treatment alternatives.
- Completes clinical documentation for client records promptly, adhering to relevant policies and procedures.
- Implements exposure control measures as outlined in the VNA Health Center Infection Control Manual.
- Familiarizes with emergency procedures and knows the locations of emergency supplies in all clinical settings.
- Upholds professionalism and maintains client confidentiality in all practices at VNA Health Center.
- Participates in quality improvement initiatives, including clinic observations, chart audits, outcome assessments, client satisfaction surveys, clinic workflow analysis, proficiency testing, and quality management meetings.
- Stays informed about VNA Health Center policies, procedures, and protocols, assisting in the review and development of standing orders and guidelines to ensure compliance with state and federal regulations and current medical standards.
- Provides consultative support as needed for nursing, laboratory, and clinic staff.
- Conducts educational sessions for staff and patients.
- Identifies and engages in continuing education opportunities to enhance professional skills and ensure high-quality care.
- Participates in quality assurance activities.
- Provides on-call support as scheduled.
- Prescribes medications within the scope of practice.
- Remains knowledgeable about VNA Health Center grant programs and regulatory guidelines, including Family Planning, Breast and Cervical Cancer Screening, and other relevant health requirements.
