Key Responsibilities
  • Provide direct patient care to adult and geriatric patients with a variety of physical and behavioral health needs.
  • Administer medications and treatments as prescribed by physicians and other healthcare professionals.
  • Conduct vital signs, monitor patient conditions, and report any changes or concerns to RNs and physicians.
  • Assist with patient education, discharge planning, and follow-up care.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Participate in quality improvement initiatives and contribute to a culture of safety and excellence.
Requirements
  • Graduate of an accredited LPN program.
  • Active, full, current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth of the United States or the District of Columbia.
  • At least 6 months of qualifying experience as an LPN or LVN, or completion of at least 24 months of approved school of at least 24 months duration.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare teams.
