Physical Therapist PRN

Job Summary

Children's Health is seeking a per diem / prn Physical Therapist for a per diem / prn job in Grapevine, Texas. As a Physical Therapist PRN, you will participate in the evaluation and treatment of patients based upon current principles and practices of Physical Therapy.

Responsibilities
  • Provide direct Physical Therapy evaluations and re-evaluations, including assessment of prior and current level of function, setting individualized goals and making discharge recommendations in accordance with standards of practice.
  • Provide direct PT treatments according to standards of practice, including patient and family education, progression of treatment plan and implementation of appropriate therapeutic interventions.
  • Report on patient progress through appropriate written documentation including evaluation forms, progress notes, education forms.
  • Collaborate with physicians, patients, family and other members of the health care team relative to patient treatment and progress as well as other affiliated agencies with regard to effective discharge planning.
Requirements
  • At least 1 year job related experience.
  • Two-year Associate's degree or equivalent experience.
  • License to practice physical therapy in the State of Texas.
  • Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements.
