Key Responsibilities:
- Coordinate treatment and implement services for assigned clients, including outreach visits, as a member of a multi-disciplinary Early Childhood Services team.
- Provide outreach services to children and families, consulting with team members in the development of individualized plans and multidisciplinary services for assigned clients.
- Maintain client records completely and accurately, including IFSPs with full assessment of client functioning and goals, documentation of client progress, and records of required contact with collaterals and family members.
- Complete billing documentation according to the schedule established by the company, adhering to professional standards and guidelines.
- Keep supervisor informed of progress of all clients, meeting direct service requirements based on part-time or full-time status.
Requirements:
- Current licensure as a Physical Therapist by the Massachusetts Board of Allied Health Professions.
