Overview

Fresenius Medical Care, a leading provider of kidney care services, is seeking an experienced Nurse Care Coordinator to join our team.

Job Description

This role involves coordinating patient care and collaborating with healthcare professionals to ensure seamless delivery of services. As a key member of our team, you will be responsible for:

  • Providing direct patient care that regularly involves heavy lifting, moving of patients, and assisting with ambulation.
  • Initiating and coordinating communication with FKC and Non-FKC dialysis providers and appropriate contracted facility personnel as needed to provide continuity of patient care.
  • Administering medications as prescribed and documenting appropriate medical justification and effectiveness.

Salary and Benefits

We offer a competitive salary range of $83,000 - $110,000 per year, depending on experience, plus benefits including health insurance, retirement plans, and paid time off.

Required Skills and Qualifications

To be successful in this role, you will need:

  • A registered nurse (RN) degree from an accredited school of nursing.
  • At least 6 months of experience in acute dialysis as a RN.
  • Strong nursing skills and excellent communication abilities.
  • Ability to work effectively in a fast-paced environment.
