We are seeking a skilled Speech Language Pathologist to join our team at Peachtree Centre in Gaffney, SC. This 150-bed skilled nursing facility offers a state-of-the-art rehab gym and community reintegration programs to facilitate our residents' highest level of function.

Responsibilities
  • Conduct thorough assessments of residents to determine swallowing and communicative deficits and equipment needs.
  • Develop and implement individualized treatment programs based on initial evaluations and containing achievable functional goals.
  • Comply with evaluation, treatment, and documentation guidelines, and maintain timely and accurate departmental records.
  • Enter treatment and progress notes in each treatment session's resident record.
  • Revise resident treatment programs as needed, based on resident performance.
  • Document the course of treatment in a clear, concise discharge summary using a prescribed format.
  • Orient residents to program services and treatment procedures appropriate to their needs.
  • Work with nursing staff, residents, and families to maximize residents' use of functional abilities.
  • Instruct residents and family/caregivers about therapeutic procedures to be continued by the resident at home following discharge.
  • Assist in the referral process when a resident requires additional services following discharge.
  • Attend and participate in rehabilitation team conferences regarding resident progress, problems, or needs.
  • Attend and participate in other department/facility meetings as required.
  • Access continuing education opportunities appropriate to discipline and responsibilities to maintain clinical currency.
  • Participate in and provide in-service education sessions.
  • Participate in the quality improvement process by responding appropriately to results of medical record audits, resident satisfaction surveys, safety activities, and regulations.
  • Act in compliance with Ahava Therapy Services' regulatory and professional standards and guidelines.
  • Adhere to facility policies and procedures and participate in facility quality improvement and safety programs.
  • Perform other duties as assigned and required to provide quality care to residents.
Qualifications
  • Master's Degree in Speech and Language Therapy.
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP).
  • Valid state Speech Language Pathology licensure.
  • BLS CPR certification.
