Role Overview:

The Clinical Nurse Coordinator plays a vital role in ensuring patient-centered care by:

  • Coordinating comprehensive care plans and managing patient stays.
  • Facilitating effective discharge planning in collaboration with the healthcare team.
  • Conducting thorough assessments of patients' psychosocial and medical needs.
  • Implementing strategies to reduce readmission risks through education and resource management.
  • Documenting care coordination efforts and maintaining clear communication with all stakeholders.

This position requires a graduate of an accredited nursing program, with a preference for a Bachelor's Degree, and a minimum of 3 years of clinical nursing experience.
