About the Role:
We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at AcuteCare Health System LLC. As a key member of our interdisciplinary team, you will provide primary medical management in collaboration with our physicians. Your expertise will be invaluable in developing and implementing participant plans of care, conducting assessments and reassessments, and evaluating participants' physical complaints. Our competitive compensation package includes an estimated $120,000 - $150,000 per year, comprehensive benefits, and opportunities for professional growth and development.

Responsibilities and Expectations:
* Provide primary medical coverage in collaboration with PACE physicians
* Conduct initial, semi-annual, and unscheduled assessments and reassessments in collaboration with the physician
* Participate in the development and implementation of participant plans of care as part of the interdisciplinary team
* Function as a member of the interdisciplinary team, maintaining regular attendance at meetings and collaborating on care planning decisions
* Integrate primary care treatment plans into the plan of care developed in collaboration with the interdisciplinary team
* Communicate participant changes and collaborate on care delivery with the team
* Evaluate and treat participants during acute illness and manage chronic conditions in collaboration with the physician(s)

Requirements:
* Master's degree in clinical nursing from an accredited Nurse Practitioner program
* 3-5 years of nursing experience, with Nurse Practitioner experience preferred and 1 year of experience with a frail or elderly population
* RN licensure in the perspective state and a Family certification from the Nurse Practitioner program
