We are looking for a talented Therapy Care Coordinator to join our team at HexaQuEST Health, Inc. As an occupational therapist or physical therapist, you will be responsible for providing high-quality patient care services to patients of all ages. This includes evaluating/assessing patients, establishing and implementing a plan of care, assessing the effectiveness of treatment, and educating patients/caregivers.

Key Responsibilities:
  • Evaluate and assess patients to determine their needs and develop effective treatment plans.
  • Establish and implement treatment plans that meet the unique needs of each patient.
  • Assess the effectiveness of treatment and make adjustments as needed.
  • Communicate effectively with physicians, staff, and hospital leadership.
Requirements:
  • A Bachelor's Degree or higher from an accredited Occupational Therapy program or Degree from an accredited Physical Therapy program.
  • Current BCLS certification.
  • Current unrestricted license as an Occupational Therapist in the State of Colorado OR Current unrestricted license as a Physical Therapist in the state of Colorado.
  • Current unrestricted certification from Hand Therapy Certification Commission and recertification every 5 years.
Compensation:

This position offers a competitive hourly pay range of $*** - $*** per hour, depending on education and experience. Additional forms of discretionary compensation may be paid to eligible employees based upon organizational and individual performance.

Benefits:

Eligible employees participate in an attractive benefits package, including medical, dental, vision, paid time off, education assistance, 403(b) with employer matching, and more. Benefits are based on employment status and will be discussed during the hiring process. Our estimated salary is around $64,000 - $81,000 per year, considering the cost of living in Colorado.
