Key Responsibilities- Provide Clinical Services: Deliver clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare professionals.
- Conduct Initial and Comprehensive Evaluations: Make initial and comprehensive nursing evaluation visits, determine primary focus of care, develop the plan of care within state-specific guidelines, and submit accurate, complete, and timely documentation.
- Evaluate and Re-Evaluate Patient Needs: Regularly evaluate and re-evaluate the patient's nursing needs, ensuring they meet home health eligibility and medical necessity guidelines.
- Develop and Implement the Plan of Care: Initiate, develop, implement, and make necessary revisions to the plan of care in collaboration with the physician and other healthcare professionals involved in care.
- Current RN Licensure: Possess current RN licensure in the state of practice.
- CPR Certification: Hold current CPR certification.
- Reliable Transportation: Have access to a dependable vehicle or public transportation.
