Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team at Jacobi Medical Center, a renowned teaching hospital affiliated with the Albert Einstein College of Medicine. As a Nurse Practitioner, you will be responsible for providing comprehensive healthcare services to patients, including preventive and supportive care, health education, and disease management.

Key Responsibilities
  • Provide continuous comprehensive medical and nursing care to patients and families, including preventive and supportive health care services.
  • Manage health problems and coordinate healthcare for various patient populations, identifying and diagnosing illnesses and physical conditions, and implementing therapeutic and corrective measures.
  • Develop and maintain partnerships with community-based organizations and make referrals to consulting physicians and/or other healthcare professionals and community agencies, as necessary.
  • Evaluate and interpret patient data to plan and implement appropriate nursing interventions.
  • Develop, implement, and evaluate patient plans of care, determining effectiveness and modifying as needed to achieve medical and health goals.
  • Provide education and counseling to patients and families regarding health status, treatment, and progress, medication management, disease prevention, and health promotion, and record and maintain relevant documentation.
  • Participate in internal/external quality assurance/performance improvement activities, programs, and training, as required.
Requirements
  • A valid New York State license and current registration to practice as a Registered Professional Nurse.
  • A current certificate as a Nurse Practitioner issued by the New York State Department of Education (NYSED).
  • A current certification issued by the State Education Department to prescribe medication.
Preferred Qualifications
  • BCLS, ACLS, PALS certifications.
  • Previous ED experience.
  • Proficient in English.
