Job Overview

As a Care Continuum Specialist with Ascension St. Vincent in Indiana, you will play a vital role in providing seamless patient care throughout the healthcare continuum. This rewarding career offers a competitive salary of $60,000 - $80,000 per year, depending on experience.

About the Role

This position is responsible for coordinating patient care and communicating with patients, families, physicians, and healthcare team members. You will coordinate patient scheduling, maintain required documentation, and support standard patient access initiatives across the healthcare continuum. Your goal will be to provide exceptional patient experiences by navigating them through the healthcare system and connecting them with available community support services.

Requirements

To be successful in this role, you will need a high school diploma equivalency with 2 years of cumulative experience or an Associate's degree/Technical degree or 4 years of applicable cumulative job-specific experience. Preferred qualifications include licensure/certification as a Licensed Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board or education in a related field. Mammography Tracking experience is also highly desirable.
