Job Summary:

The Admissions Screener will provide critical support to the Continuum of Care department in coordinating the admissions process, clinical information, and disseminating resources for admission referrals for Fred and Harriet Taylor Nursing Home, TCU, and Acute Care Rehab. This role requires a highly organized and detail-oriented individual with excellent communication skills to ensure seamless coordination and timely admissions.

Key Responsibilities:
  • Locate admission referrals by collaborating with Arnot Health case managers and other referring facilities.
  • Complete pre-admission screenings accurately and conduct patient/family interviews for each assigned referral, traveling to various locations as needed.
  • Conduct pre-admission interviews of patients and families, including pre-placement preparation for those deemed suitable for admissions.
  • Consult with referring facility case managers, social workers, discharge planners, and physicians to ensure appropriateness of referrals.
  • Provide support and assistance to patients and families prior to admission or referral elsewhere.
  • Confirm insurance coverage for each patient and obtain prior approval or authorization as necessary.
  • Maintain a working knowledge of services and resources available in all facilities and post-acute care levels.
  • Schedule family meetings and healthcare team meetings as requested or needed to ensure proper payment to the facility.
  • Fax forms, information records, and other documents to facilities and community resources within HIPAA guidelines.
  • Complete documentation related to clinical information and other assigned items.
  • Effectively handle all phone contact with efficiency and courtesy.
  • Makes decisions regarding appropriateness of admissions with input from the interdisciplinary team.
  • Distribute and ensure all necessary departments have reviewed admission documents as appropriate.
  • Act as a patient advocate and resource as needed.
  • Conduct educational meetings, open houses, and in-services for referral providers to promote the nursing home and its objectives.
  • Distribute facility brochures and informational materials to referral providers.
  • Attend community functions as requested as a representative of the facility.
  • Oversee TCU admission screening staff and fill in as needed.
  • Attend all annual mandatory educational programs as required by position.
  • Employee understands and demonstrates the importance of satisfying customer/patient needs by interacting with them in a friendly and caring way, being attentive to their needs, and taking the initiative to maintain communication to provide a secure and pleasant experience within Arnot Health.
  • Has working knowledge of Corporate Compliance regulations and promptly reports any potential Corporate Compliance violations to the Corporate Compliance Officer.
  • Maintains continuing education through seminars, medical meetings, bulletins, professional journals, in-services, and JET classes.
  • Familiar with and demonstrates Continuous Quality Improvement principles and participates in the process as appropriate, focusing on meeting customer service needs of internal and external customers.
  • Possesses the highest ethical standards with respect to discretion and regard for confidential information.
  • Responsible for attending all annual mandatory educational programs as required by position.
  • In order to provide the highest quality care to our patients and residents, individuals may be required to work hours beyond those normally scheduled. Work schedules may be subject to change based on the needs of the department.

Requirements:


Current New York State licensed Registered Nurse, LMSW, or Master's Degree with relevant experience is required. Excellent verbal and written communication skills. Qualities of maturity, diplomacy, and ability to work well with others required. Demonstrated knowledge of NYS Medicaid & Medicare Programs, MDS-based Case Mix Reimbursement systems, and documentation requirements. Demonstrated willingness to uphold the vision and mission of Arnot Health by following the core values of Integrity, Teamwork, and Passion for Excellence and Reverence for the individual is required. Demonstrates strong clinical and supervisory skills, ability to work independently, and be highly motivated. Discharge planning experience recommended. Must have own vehicle. Must have current NYS driver's license, car insurance, and a good driving record. Experience: Minimum 5 years experience. CARDIOPULMONARY RESUSCITATION (CPR) REQUIREMENTS: Basic Life Support (BLS) required within 90 days of hire date. PHYSICAL DEMANDS: Routine office jobs. Works in comfortable positions. Flow of work intermittent. EXPOSURE CATEGORY: Category II. Tasks that involve no exposure to blood, body fluids, or tissues. But employment may require performing unplanned Category I tasks
