Job Title: Nurse Practitioner/Primary Care Provider

WelbeHealth PACE (All-Inclusive Care for the Elderly) program offers seniors the opportunity to continue living in their homes and communities. Our comprehensive medical services aim to treat the whole person, not just symptoms. We employ a collaborative interdisciplinary team approach to evaluate and guide participant care, aligning with our values, team culture, and mission.

Key Responsibilities:
  • Conduct comprehensive assessments, including initial, semi-annual, and unscheduled evaluations, to provide continuous medical coverage to participants.
  • Collaborate with physicians and the interdisciplinary team to develop care plans for each participant.
  • Work with physicians and the interdisciplinary team to ensure effective care coordination and discharge planning from acute or long-term settings.
  • Maintain care of participants in nursing homes, making regular visits as needed.
  • Oversee and ensure effective care coordination with WelbeHealth physicians, community-based PACE physicians, and the interdisciplinary team.
  • Review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements.
  • Participate in rotating night and weekend call.
Requirements:
  • Graduation from an accredited school of nursing.
  • Certificate as a Nurse Practitioner with prescriptive authority.
  • RN state license.
  • Minimum of two years of experience working with the senior population.
  • Current knowledge of chronic care/geriatric issues and best practices.

Preferred Qualifications:
  • Certified as a Nurse Practitioner with prescriptive authority.
  • Licensed in California.
  • Geriatrics experience (preferred).
  • 2 years of NP experience.
