Key Responsibilities- Provide direct care and assist with care as directed and consistent with your scope of practice and competency.
- Initiate the SBAR process and collaborate with the RN for assessment and recommendations.
- Make rounds with physicians and other team members, as needed.
- Participate in resident evaluations and care planning activities, reviewing and revising residents' assessments and care plans as needed.
- Coordinate residents' care activities and communicate changes to other team members and resident family/significant other.
- Administer medications and treatments, and oversee Medication Aide/Tech in medication/treatment administration.
- Review, transcribe, communicate, and implement physician orders, obtaining RN signature as required by state regulations.
- Communicate with resident's physician and assist with emergencies, administering cardiopulmonary resuscitation as needed.
- Document care performed and observations of resident status in the clinical record promptly, utilizing standard clinical documentation guidelines.
- Graduate of accredited LPN School of Nursing.
- Current valid LPN license in the state employed.
- Valid CPR certification.
- Six months experience in a long-term care environment preferred.
We offer a competitive hourly rate of $29.00 for new graduates, as well as promotable opportunities and a supportive work environment.
Please note that all employees of Colville Health are required to submit and be cleared to work in the facility per each state's specific background check requirements prior to contact with patients/residents.
