Job Summary

We are seeking a skilled Home Health Assistant to join our team at Kaiser Permanente. As a Home Health Assistant, you will play a vital role in providing high-quality care to our patients in their homes.

Key Responsibilities
  • Provide direct nursing services and care to patients in their homes, as delegated by the home health Registered Nurse (RN) and/or physical/speech/occupational therapist (PT/ST/OT).
  • Assist patients with prescribed home exercise programs and provide simple household chore services essential to healthcare at home.
  • Observe and document patient status and care/service furnished, and report any changes in body function to the RN or PT/ST/OT.
  • Educate patients and caregivers in appropriate self-care techniques and participate in multidisciplinary team conferences.
  • Maintain productivity levels outlined by the Home Health Manager/Director and participate in performance improvement activities.
Requirements
  • Minimum one (1) year community health and geriatric nursing experience.
  • Employed in community health service to geriatric population within the last twenty-four (24) consecutive months.
  • Licensed Practical Nurse (Hawaii) and Basic Life Support from American Heart Association.
