Job Summary:

We are seeking a compassionate and skilled LPN to join our team of healthcare professionals. As a Home Health LPN, you will be responsible for providing high-quality patient care, conducting thorough assessments, and developing personalized care plans.

Key Responsibilities:

  • Conduct thorough patient assessments to identify care needs and develop effective care plans
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans
  • Participate in ongoing education and training to stay current with best practices and industry developments

Requirements:

  • Current LPN license as required by state
  • 1 year of experience in an acute care or equivalent setting required
  • Prior Home Health experience preferred but not required
  • CPR certification with American Heart Association or America Red Cross
  • Positive attitude, dedication to quality patient care, strong communication skills, and reliable transportation
