OBJECTIVE:
Deliver nursing care and services adhering to the standards and scope of nursing practice as defined by state regulations, policies, and protocols. Uphold a positive, empathetic, and professional demeanor towards all individuals at all times, with a strong emphasis on patient safety.
Key Responsibilities:
ESSENTIAL FUNCTIONS:
- Assess patient health and provide fundamental care utilizing the nursing process within the LPN/LVN scope of practice to achieve care plan objectives.
- Exemplify expectations related to customer service and demonstrate urgency regarding patient safety.
- Collaborate with team members to ensure that patients' physical, biopsychosocial, age, developmental, and cultural needs are met, addressing and resolving any customer complaints as necessary.
- Ensure patient comfort by implementing individualized nursing interventions outlined in the care plan.
- Apply appropriate strategies to advance patient wellness and maintain health.
- Engage in discussions regarding care with patients and attentively listen to their concerns.
- Report any changes in patient status and concerns to registered nurses and physicians.
- Communicate effectively and professionally with patients, visitors, colleagues, and others involved in the patient's care.
- Administer medications safely in accordance with established policies, physician orders, and current practices.
- Maintain professional nursing standards in line with facility policies, external governing bodies, and performance improvement standards.
- Ensure thorough and accurate documentation of patient symptoms, responses, and progress.
ADDITIONAL FUNCTIONS:
Perform other duties and responsibilities as assigned.
Qualifications:
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
Graduation from an accredited nursing program is required.
Preferred: One or more years of nursing experience in a clinical setting relevant to the services provided.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
Current LPN/LVN license as mandated by state regulations.
CPR and de-escalation/restraint certification required (training provided upon hire).
First aid certification may be required based on state or facility guidelines.
