Job Summary:
This position provides comprehensive nutrition services and education to dialysis patients and authorized caregivers in a clinical setting, focusing on maximizing the patient's nutritional well-being in a chronic-care facility for end-stage renal disease patients. The successful candidate will work in accordance with LifeBridge Health policies, procedures, and guidelines, as well as OSHA, CMS, federal, state, and local regulations.
- Assessment and Intervention: Conduct initial assessments within 30 days of admission or 13 treatments, whichever is later, and re-assessments within 3 months of initial assessment, annually for stable patients, and monthly for unstable patients.
- Interdisciplinary Care Plan: Participate in care plan development based on assessment or re-assessment, with a 15-day turnaround time after assessment completion, annually for stable patients, and monthly for unstable patients.
- Nutrition Education and Counseling: Provide individualized Medical Nutrition Therapy, including meal plans, and nutrition education and counseling based on patient needs, including nutrient requirements, knowledge, and abilities, medical, economic, and social situations.
- Monitoring and Revision: Monitor nutritional status, laboratory values, dialysis adequacy, adherence to diet, and response to nutrition-related medications and therapy, revising and updating nutritional recommendations and care plans as needed.
Requirements:
- Education: Bachelor's degree in nutrition, dietetics, or a similar area.
- Registration: Registered Dietitian status with the Commission on Dietetic Registration.
- Licensure: State licensure in Maryland or the state where the facility is located.
- Experience: Minimum of two years' professional work experience in clinical nutrition as a Registered Dietitian, with a preference for five years of experience in dialysis settings.
