Job Summary

We're seeking an Occupational Therapist to join our team in Springfield, IL. As an Occupational Therapist, you'll work with patients to develop and implement personalized plans of care to help them achieve their goals and improve their quality of life.

Responsibilities
  • Evaluate patients to determine their rehabilitation needs and develop plans of care in accordance with our vision and regulatory guidelines.
  • Implement and develop plans of care to ensure quality and continuity of care and proper discharge planning.
  • Communicate significant findings, problems, changes in condition or environment to the appropriate supervisor, physician, and other team members involved with patient care.
  • Initiate plans for patient safety, using patient, family, and community resources.
  • Verify plans of care prior to each visit and provide care according to physician's orders, assessment data, and established standards and guidelines.
Requirements
  • Graduate of an accredited Occupational Therapy program.
  • Current state license as an Occupational Therapist.
  • 1+ year experience as an occupational therapist in a clinical care setting.
  • Home Health experience is preferred.
  • Effective verbal and written communication skills.
  • Ability to travel to meet patient needs within branch coverage area.
