Job Title: Full-Time Hospice Registered Nurse Case Manager

We are seeking a compassionate and skilled Registered Nurse to join our team as a Full-Time Hospice Registered Nurse Case Manager. As a key member of our healthcare team, you will provide high-quality patient care and coordinate services to meet the needs of our patients and their families.

Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
  • Coordinate with healthcare providers and community resources to ensure seamless care transitions.
  • Collaborate with interdisciplinary teams to develop and implement care plans that meet the unique needs of each patient.
  • Document patient progress and outcomes in accordance with regulatory requirements.
Requirements:
  • Current RN licensure in the state of Oregon.
  • Minimum of two years of experience in hospice or home healthcare nursing.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
